Admission Information

Health Information Practices

Application for admittance

Admission to Brooklawn is based on referrals from one or more mental health professionals. To be admitted to Brooklawn's residential program a child must:

  • Have a documented history of emotional disabilities.
  • Be male.
  • Be 6 to 17 years of age.

A multi-disciplinary admissions committee meets twice a week to decide which children to accept and when they can move in. The committee's job is to select those children who will benefit most from the treatment Brooklawn offers.

Many factors influence the committee's decision and the information comes from the admission process.

Step 1:

Complete the application and fax it to Brooklawn at (502) 479-5609. Please include any additional documentation from previous psychiatric services or placements. This information will be reviewed by the Admission, Transfer and Discharge Committee to determine whether the applicant would be an appropriate candidate for placement at Brooklawn. We usually respond within three working days, notifying you whether or not your applicant has been accepted or will need to proceed to Step 2. If an applicant is accepted at this stage he will either be placed on a waiting list or given a probable move-in date.

Step 2:

Interview of both the guardian and child. We prefer to have this take place on the Brooklawn campus so that the child and guardian can become more familiar with our program and facilities. Brooklawn admissions staff will travel to interview applicants if necessary. We usually report within three working days, notifying you whether or not your applicant will proceed to Step 3.

Step 3:

Once a child has been interviewed, consideration of the applicant will go back to the Admission, Transfer and Discharge Committee. At the conclusion of this meeting, applicants qualifying for admission will either be placed on a waiting list or given a probable move-in date. Please remember that, although the admission process is complicated and time consuming, it usually results in appropriate and effective treatment for those children who are admitted.
