IMPACT Plus/Outpatient Services…


  Brooklawn provides IMPACT Plus outpatient mental health services for children and adolescents who are experiencing ongoing behavioral and emotional problems.

IMPACT Plus services can be provided to young people in the home, in the community, or in our offices, and are designed to help families prevent out-of-home placements and/or inpatient hospitalizations. Brooklawn’s IMPACT Plus outpatient therapy services assist in improving specific behaviors, and increasing overall family functioning.

IMPACT Plus’ case management service connects families to vital community resources, and helps to facilitate a team treatment approach.

Brooklawn’s IMPACT Plus outpatient program includes the following services:


Targeted Case Management Services

Our Case Managers organize mental health services, individual providers, and community resources for the family, and facilitate a team approach to treatment.

Individual and Family Therapy

Master’s Level clinicians provide in-home, in the community, school, or in-office therapy that can address a wide variety of behavioral and emotional problems including trauma, conduct disorders, hyperactivity, depression, school related issues and grief and loss.

Therapeutic Child Support Counselor

An IMPACT Plus Counselor will provide therapeutic support to the young person and family focused on improving independent living, social, interpersonal, and behavior management skills.

 

Brooklawn’s IMPACT Plus are offered to young people under the age of 21 who:

  • Are Medicaid eligible
  • Meet IMPACT Plus criteria
  • Are diagnosed with an emotional or behavioral disorder that disrupts functioning in the home, school and/or community.
  • Less intensive services are not appropriate to meet child’s need or are unavailable.
